Summary: The Direct Support Professional 2 will support and assist in Level 2 residential programs. <li>Provide support and assist individuals with skills training in all activities of daily living.</li>
<li>Create opportunities and provide support to individuals to make decisions to develop their skills and foster independence.</li>
<li>Provide services to individuals according to DDS, DMH, MassHealth, HUD, DPH, CARF, and all applicable regulatory standards, as well as Bridgewell policies and procedures.</li>
<li>Provide support and assist individuals with all medical, clinical, and financial needs.</li>
<li>Transport and accompany individuals as needed to and from medical, recreational, and other activities outside the program.</li>
<li>Maintain required documentation according to DDS, DMH, MassHealth, HUD, DPH, CARF, and all applicable regulatory standards as well as Bridgewell policies and procedures.</li>
<li>Assist individuals with accessing resources and recreational opportunities in the community.</li>

As a Personal Care Assistant, your main focus will be ensuring our clients are safe, feel supported, and are able to remain at home by providing assistance with everyday needs. This involves following individualized care plans, reporting any significant changes promptly, and consistently demonstrating care and compassion during every interaction. The successful candidate will ideally have proven experience in the home care, healthcare, and/or customer service industry.

What you'll be doing:
<ul>
<li>Assist clients with activities of daily living, such as bathing, grooming, dressing, and toileting.</li>
<li>Assist with mobility, transfers, and ambulation.</li>
<li>Assist with preparing meals and feeding.</li>
<li>Provide medication reminders to help clients stay on schedule.</li>
<li>Provide companionship and emotional support.</li>
<li>Ensure accurate and timely documentation.</li>
<li>Assist with light housekeeping.</li>
<li>Run errands as needed.</li>
</ul>

What we're looking for:
<ul>
<li>Previous caregiving experience preferred.</li>
<li>Ability to pass a pre-employment physical and TB test.</li>
<li>Verification of professional references.</li>
<li>Willingness to travel up to 30 minutes.</li>
<li>Reliable transportation.</li>
<li>Must be 18 years of age or older.</li>
<li>Must be able to communicate effectively in English.</li>
