GENERAL SUMMARY
Provides direct 1:1 instruction and individualized support to adults with developmental disabilities and/or acquired brain injury living in their own homes. The role focuses on promoting independence through skill development, social engagement, and community access. The Direct Support Professional (DSP) actively supports individuals in building meaningful routines, accessing healthcare, and participating in community and recreational activities. This position involves ongoing collaboration with the individual, team members, supervisor, families, clinical consultants, and external partners.
 
ESSENTIAL JOB FUNCTIONS*
Ø Provides individualized personal care assistance as needed, promoting dignity and independence.
Ø Implements goals and objectives as outlined in the Individual Service Plan (ISP).
Ø Maintains accurate documentation of services provided, including medical and daily records, in compliance with agency and regulatory standards.
Ø Supports individuals in accessing medical care, including attending appointments and ensuring completion of required documentation by healthcare providers.
Ø Provides instruction in medication routines and, with certification, administers medications in compliance with state and agency guidelines (MAP).
Ø Promotes skill-building in areas such as meal preparation, grocery shopping, menu planning, budgeting, and bill paying.
Ø Teaches and reinforces daily living skills including cleaning, organizing, and maintaining a sanitary household.

Ø Maintains regular, open lines of communication and a positive rapport and proactive attitude with staff, families, supervisors, and Department of Developmental Services (DDS) and Massachusetts Rehabilitation Commission (MRC) representatives.
Ø Reports any incidents or concerns promptly and in accordance with agency and regulatory procedures.
Ø Acts as a mandated reporter for suspected cases of abuse and/or neglect as required by law.
Ø Attends required trainings, meetings, and certifications to remain current with agency and regulatory expectations.

Essential Duties and Responsibilities:

<li>Provide support and assist people in our services with skills training in all activities of daily living during the day, evening and/or overnight hours (depending on the shift the employee is assigned).</li>

What you’ll do:

Community Bridges is looking for 1 full-time Job Coach, Direct Support Professional for our Supported Employment Program in Concord, NH! 

We are a non-profit agency founded in 1982 with a mission to advance the integration, growth, and interdependence of people with developmental disabilities within their own communities. Building on the strengths of over 1,100 individuals in the greater Capital Area and their families, we work to identify barriers of access to find creative solutions to develop meaningful community engagement.

The Job Coach has the responsibility to provide on the job assistance to those with barriers to employment. The position includes assessing vocational abilities and safety skills as well as coordination of career activities including; interviewing, filling out job applications, resume writing, assistance with job placement as needed as well as primarily on the job supports. On the job supports include role modeling, problem solving, interpersonal skills, and mentoring the individual to create success on career paths.

Qualifications:

<li>Help design plans and work with employee's onsite to create a successful fade program and be a resource to the job site for ongoing support.</li>
<li>Provide supports while considering the preferences of individuals. Actively encourage choice making.</li>
<li>Assist in developing the Individual Service Plan, provide information for assessments, develop implementation strategies for goals/objectives, and monitor the delivery and quality of supports.</li>
<li>Complete all necessary documentation as specified by both the department handbook as well as the Individual Service Plan. These include but are not limited to, monthly Progress notes, daily AWARDS notes, incident reports, data collections, probes, and individual data.</li>
<li>Demonstrate an awareness of and respect for each person's legal, civil, and human rights- including maintaining confidentiality of information regarding the individual. Ensuring the respect and dignity of persons with developmental disabilities as well as reporting any safety, health or human rights concerns immediately to the Supervisor.</li>
<li>Attend team meetings, agency meetings, trainings and seminars as assigned.</li>
<li>Perform all delegated responsibilities, which may include filing, updating data collection tasks, and other activities related to the provision of supports and services.</li>

Work Environment and Physical Demands:

Walking and standing required frequently with occasional stair climbing. Emergency behavioral intervention in accordance with behavioral intervention techniques and individual behavioral plans.
