Summary: The Direct Support Professional 2 will support and assist in Level 2 residential programs. Staff will help individuals who need daily intervention, supervision, and skills training in activities of daily living while managing within a home environment and in the community. Essential Duties and Responsibilities:

<li>Provide support and assist individuals with skills training in all activities of daily living.</li>
<li>Create opportunities and provide support to individuals to make decisions to develop their skills and foster independence.</li>
<li>Provide services to individuals according to DDS, DMH, MassHealth, HUD, DPH, CARF, and all applicable regulatory standards, as well as Bridgewell policies and procedures.</li>
<li>Provide support and assist individuals with all medical, clinical, and financial needs.</li>
<li>Transport and accompany individuals as needed to and from medical, recreational, and other activities outside the program.</li>
<li>Maintain required documentation according to DDS, DMH, MassHealth, HUD, DPH, CARF, and all applicable regulatory standards as well as Bridgewell policies and procedures.</li>
<li>Assist individuals with accessing resources and recreational opportunities in the community.</li>

Duties and Responsibilities May Include (depending on Client Service Plan):
<ul>
<li>Offering companionship and emotional support </li>
<li>Housekeeping </li>
<li>Assists clients with transfers and ambulation including use of cane, walker, and wheelchair or Hoyer Lift (specific to client) </li>
<li>Assists clients with bathing, dressing and grooming </li>
<li>Plan, prepare and serve meals, with assistance from the clients (when they are able)</li>
<li>Assists clients with toileting, including use of bedpan, or toilet </li>
<li>Report on any unusual events or occurrences to the administrator </li>
<li>Act quickly and responsibly in cases of emergency </li>
<li>Completes all other duties within the Client Service Plan</li>
