Position Overview:

Core Responsibilities:
Establish a Game Plan (Treatment Plan) with the client and assist them in achieving their goals.

<li>Attend court proceedings.</li>

Administrative Responsibilities:

<li>Virtual Gateway (computer-based reporting system).</li>
<li>Monthly and weekly reports and other paperwork requirements.</li>
<li>Attend all staff meetings, clinical consultations, and client reviews.</li>
<li>Attend weekly supervision.</li>

Youth, Purpose &amp; Partnership (YPP) is a year-round leadership development, civic engagement, and workforce development program founded in 1995, as Youth &amp; Police in Partnership, in response to rampant youth homicides and gang violence afflicting Boston. In 2023, led by youth voice, YPP redesigned and refined programming, deepening emphasis on building capacity of young people of color in media production skills and creating digital storytelling projects, opening access to careers in Boston’s burgeoning STEM economy. YPP is designed to build a digital creative community led by and for young people of color. Through YPP young people build and grow communications skills, gain work experience and credentials, and establish and progress towards personal and professional goals. The program is divided into two groups based on experience and age.
Community Peer Leaders programming focuses on developing young people’s research and storytelling skills through photo and audio storytelling of the history and current status of the neighborhood. The Peer Leaders will need to convey in their design why YPP exists. The modules will need to be both engaging and include historical contexts of Boston and this neighborhood while expressing current challenges people in this neighborhood face. These modules will be used for the Fall of 2023, both recruitment and training of the next group of YPP community peer leaders. 
Dialogue Peer Leaders programming includes developing the skills to facilitate an intergenerational dialogue and will build the technical skills of video storytelling. The Dialogue Peer Leaders will use video to engage a broader audience about an issue they care about in the community and to share the dialogue they host and facilitate to a broader audience.

Position Summary:
The Therapeutic Training &amp; Support Staff at River Valley Counseling Center (RVCC) delivers in-home, community-based therapeutic support to children, adolescents, and families. The TT&amp;S works collaboratively with clinicians, maintains timely and accurate documentation, and demonstrates cultural competence while helping families access needed resources.
 
Key Responsibilities:

Provide high quality in home therapeutic training and support services to children/adolescents and their families
Provide in-home services (home, clinic and community settings) to youth/family at least twice per week or as clinically indicated 
Assist youth and family in obtaining/maintaining linkages with outpatient treatment by assisting with attending OP appointments
Provide care coordination and referrals to services and agencies as appropriate
Support parent and youth skill-building, mentoring and behavior management training as part of the treatment plan
Provide client transportation as needed to address care coordination and treatment needs
Maintain a direct service productivity of 70% of available time
Write progress notes written in manner that reflects progress being made toward treatment goals and submit progress notes within 24 hours of service delivery
Support clinician with intake paperwork, Safety Plan and discharge planning
Attend in-services or meetings as assigned or directed.

Education Level - Bachelor’s degree in a related field.
Years of Experience - At least one year of experience in the human service field, skilled in parenting education and experience working with children and families, broad knowledge of mental illness diagnoses, behavioral modification techniques/coping mechanisms and familiarity with internet, word processing and email systems.

Center for Life Management (CLM) provides Southern New Hampshire's most comprehensive array of outpatient mental health services for children, youth and families. Our multi-disciplined treatment teams work with individuals ages 3 – 21 who experience mental illness and/or serious emotional difficulties.
Assertive Community Treatment (ACT) is an evidenced based practice that provides an intensive and highly integrated approach to community mental health services. ACT support helps at-risk children, youth and their families bring about positive change in symptoms and behaviors that impact school, relationships, living arrangements, and overall wellbeing. ACT reduces mental health crisis, unnecessary hospitalization, encounters with law enforcement, involvement in the juvenile justice system, and out of home residential placement. The Children’s ACT Therapist to provide outpatient clinical services including diagnostic assessment, treatment planning, crisis management, individual therapy, family therapy and community-based support. This is a full time 40 hour per week position.
 
Major Functional Areas and Description of Duties

<li>Provide individual diagnostic assessment to determine client’s strengths, needs, resources, and objectives;</li>
<li>Formulate treatment plans specifying presenting problems, objectives, interventions, and desired outcomes;</li>
<li>Complete assessments for eligibility determination for State funded services;</li>
<li>Provide therapy in the home and community, engaging natural supports as often as possible;</li>

<li>Provide set hours of billable service each week (eligible for incentive pay upon exceeding productivity expectation);</li>

What You'll Do:

<li>Document session data to track and support treatment goals. </li>

What We're Looking For:

</ul>
<ul>
<li>Willingness to obtain your BCAT credential within 90 days (we'll help!). </li>
</ul>

<li>Availability for weekday afternoons/evenings and optional Saturdays. </li>
</ul>
<ul>
<li>A valid driver's license and reliable transportation (within a 30-40 minute drive radius). </li>
</ul>
<ul>
<li>Tech-savvy with EMR systems and Microsoft Office tools. </li>

The Role:

Experience and Qualifications:

<li>Bachelor's Degree preferred, not required.</li>
<li>Physically capable of backpacking with a full backpack on uneven terrain.</li>

A Week in the Life:

<li>4 days of onsite programming with our clinical and residential team, 2 days of offsite adventure programming, and a rest and recovery day.</li>
<li>Adventure activities may include: hiking, camping, xc skiing, snowshoeing, canoeing, rock climbing, teambuilding.</li>
<li>Farmhouse activities may include: diet and nutrition, arts and crafts, therapeutic groups, community building, fun and games!</li>
<li>130 acres of woods, trails, fields and water on our property.</li>
