Responsibilities
Elwyn Adult Behavioral Health Services seeks a Recovery Support Worker (RSW) to build relationships with individuals and develop shared understandings of the individuals' goals and the steps necessary to achieve the goals.
ESSENTIAL FUNCTIONS:

<li>Reviews the Screening, Assessment, Crisis Plan, Critical Needs Plan and IAP upon completion or update completed by the LPHA.</li>
<li>In conjunction with the Integrated Team/GLE/IGLE staff, assists persons with identifying and implementing meaningful goals for their Individual Action Plan focusing on the following areas: mental health recovery education, daily living skills, substance use, relationships, independent transportation, self-help groups, exercise/wellness groups and the Recovery Learning Center (RLC), vocational &amp; leisure.</li>
<li>Demonstrates knowledge of prescribed medications, ability to monitor medications, and follows FHR state specific medication guidelines.</li>
<li>Organizes, plans, and facilitates a range of valued community activities that offer wellness/fitness, social, and cultural opportunities outside of the persons' home.</li>
<li>Offers or assists with transportation needs, supporting engagement/participation in recovery focused community resources.</li>
<li>Maintains a positive attitude and promotes an environment that is recovery based and supports the individual rehabilitation goals and interventions of consumers that support hope, healing and empowerment.</li>
<li>Works with person to identify and utilize preferred recovery tools and self care techniques (i.e. WRAP, peer groups, etc.).</li>
<li>Demonstrates appropriate therapeutic boundaries.</li>
<li>Provide face-to-face crisis intervention to aid the person in managing a crisis, providing short-term intervention to decrease risk, manage symptoms, and maintain safety.</li>
<li>Carries out routine daily tasks including but not limited to scheduling/ rescheduling or attending doctor's appointment, shopping, entitlement/housing support, court, etc.</li>
<li>Shares and encourages persons to participate in the completion of domestic tasks, assisting in the overall maintenance/upkeep of the program/supported living offering a safe clean and comfortable and safe environment for treatment to take place.</li>

<li>Assist with the development and individualizing the yearly Person Centered Planning by providing information to assist with the implementation strategies for goals/objectives and methods for monitoring the delivery and quality of supports to the individual.</li>
<li>Complete documentation requirements as specified by the Individual Service Agreement to include the scheduled data collection and monthly progress notes.</li>
<li>Will stay in compliance with all company and state required trainings which will include Relias and in person trainings as well as yearly ISA Sign Offs.</li>
<li>Report any safety, health or human rights concern immediately to the Supervisor.</li>
<li>Understand and ensure compliance with applicable state regulations.</li>
<li>Perform other duties and responsibilities as assigned.</li>

Environment and Physical Demands:
<ul>
<li>Work environment- The positions is typically in the community. You will be required to come to the office for in person trainings and meetings.</li>
<li>Physical demands- Direct Support Providers must be physically able to lift up to 50 lbs. and to provide transfers if necessary. Walking and standing required frequently with occasional stair climbing. Must be able to push wheelchairs and operate wheelchair lifts/Hoyer and barrier free lifts if required for the client. Emergency behavioral intervention must be followed in accordance with behavioral intervention techniques and individual behavioral plans.</li>
</ul>
 
Travel:
Must have access to personal vehicle during all scheduled work hours. Frequently travels with supported individuals utilizing personal vehicle. Must possess and maintain a valid NH license and safe driver's record in accordance with Community Choices standards with adequate liability insurance.
