The RN Case Manager (RNCM) is a total quality client/family advocate that coordinates all care in a
multidisciplinary method. A registered nurse who is responsible for the coordination of care of the
child and family assessing needs and coordination of appropriate clinical options. The nurse case
manager facilitates referrals to primary providers, specialists, healthcare facilities and
community resources. Responsible for direct client care including a holistic approach to the care
of children and their families. This includes educational, psychosocial, behavioral, physical,
mental and emotional care/needs of the child. The nurse will also educate the client and family
regarding immediate care needs and preventative behaviors.
Graduate of an accredited school of nursing with current RN licensure in the state of Kentucky.
BSN preferred. Basic personal computer skills and comfort with Windows Operating System. A
minimum of six months current experience as a practicing RN with pediatric experience preferred.
Current negative Tuberculosis skin test (PPD)
required. Current CPR (Basic Life Support) required.
