Adult Family Care RN:
Full time position for an RN to work as part of an interdisciplinary Adult Family Care/Shared Living team serving programs primarily within Franklin County and the North Quabbin area. The Adult Family Care and Shared Living programs provide support to individuals in need of assistance with personal care and wish to live in the community with a host family. This position will function as part of a nursing team and will be inclusive of duties and back-up to programs within the Client Services Department.
Requirements include assessment/evaluation skills, ability to develop care plans, client advocacy, ability to educate clients and caregivers be able to work well autonomously. Degree and community and/or geriatric nursing experience or previous workk with DDS/DMH preferred. Computer proficiency required. HOME CARE RN:
Full time position for an RN who will work as an active member of the interdisciplinary care management team, serving programs within Client Services Department primarily in Franklin County and the North Quabbin area. Duties include clinical assessment and eligibility determinations/redeterminations for clients and applicants in a variety of programs, including MassHealth, nursing facility and community-based long term care, and state home care. Monitors appropriateness of programs through on-site home visits as per program guidelines.
Knowledge of resources in the community-based long term care and elder network preferred. Computer proficiency and ability to work autonomously and as part of a collaborative, client-driven service team are required. Must drive and have reliable, insured transportation to travel throughout service area. Current RN license, a BS in Nursing, or AS in nursing and a minimum of 1 year of nursing in a clinical, community, or long-term care setting or a diploma RN and 2 additional years of community-based nursing are required.
